Clinician: Thanks for coming in today. On a 0–10 scale, where has your anxiety sat this week?
Client: Mostly a 7. Still waking up with that chest tightness before work standup.
Clinician: Walk me through yesterday morning.
Client: Alarm at 6:30, ruminating about the sprint demo. Skipped breakfast. Used the 5-4-3-2-1 grounding we practiced — helped for maybe ten minutes, then the worry loop came back.
Clinician: Any panic attacks?
Client: One on Thursday in the parking garage. Heart racing, dizzy, lasted maybe eight minutes. I didn't go to ER — used paced breathing.
Clinician: Sleep?
Client: Four to five hours. Nightmares about being late. Caffeine is down to one coffee.
Clinician: PHQ-9 is 11 today, GAD-7 is 15 — still in moderate range. Are you taking sertraline as prescribed?
Client: Yes, 50mg every morning. No side effects I notice.
Clinician: Let's keep CBT focus on worry postponement and behavioral activation. Any SI/HI?
Client: No. I do feel hopeless about the job sometimes, but no plan or intent.
Clinician: Plan for the week: daily worry window at 7pm for 15 minutes, three short walks, sleep hygiene checklist, continue meds, return next Tuesday.
